I'm very picky and I've personally used Amember Pro ( http://www.amember.com/ ) to manage subscriptions on one of my sites. It does everything that you're asking for - and more. Installation is included with the $179 purchase price. The only thing you'll need to do is answer questions from your subscribers - though a comprehensive list of frequently asked questions (on your site) would cut down some of that.
